I can say that was shown in training as a direct example of how thee situations are not supposed to be handled. Especially when you watch the swat team get there and then stand outside strapping on gas mask bags and other useless gear. The waiting for swat bullshit is one of the reasons Columbine was able to drag on for so long.

Now the new is to get on scene and immediately push to the area activity is, ignoring a lot of room clearing and diligent approach in favor of engaging as quickly as possible. When I first started working the policy and training was based around the idea of getting officers to pair up on scene as they arrive and quickly push in bypassing any injured or dead to drop the shooter and secure the scene for triaging casualties and investigations.
